MEDICAL – TRAVEL – PHOTO – PICK UP PERMISSION
For: all students
Print and return with CAFT form and any other relevant documents to complete the registration process.
Enrolment – Direct Deposit (CAFT) Banking form
For: all students
Print and return with Step 2 forms and a void check. All families are expected to use the Direct Deposit system for tuition payments made throughout the school year. Exemption from this policy may be requested through application to the Centre School Board.
Criminal Record Check
For: Choices Instructors, Noon Supervisors
Please return with Step 2 Forms. The Centre School will cover the cost of this application. School policy requires anyone, whether volunteer or work exchange, which is spending time alone with any group of children without staff supervision to give consent for the school to have an annual criminal record check done. This information will be kept strictly confidential. Go to http://www.pssg.gov.bc.ca/criminal-records-review/index.htm for more information.
RCMP Record Check, Driver Abstract Info
For: drivers for Choices, off land classes (i.e. gymnastics, swimming), field trips.
School policy requires anyone, whether volunteer or work exchange, who is driving students on behalf of the school to complete an RCMP Record Check. Application must be accompanied by a cover letter from the school. Please request this letter from the Administration Office. Record Check forms are to be requested from and signed in person at the RCMP station any Wednesday from 1-4pm and returned to the School Admin Office prior to driving.
Parents must also provide a copy of their drivers license and car insurance.
Change to “Non-Parent” Pick Up Information
For: anyone wishing to make changes to their “permission for pick up” information in family files.
Change to Medical Information or Medical Care Instructions
For: adding to or altering medical instructions in student files.
Notification of Temporary Absence
All families agree to pay an additional $25 per day for absences in excess of 26 school days prior to May 25 (except due to illness) to compensate for lost funding.
In the case of an extended absence (e.g. extended holiday) funding can still be received if the school continues to provide and supervise the student’s educational program and assesses student progress. Parents are responsible for informing the office and teacher 2 months in advance of such an absence. Parents are also responsible for committing time to oversee the program and sign out and return any borrowed resources, as well as submitting the student’s finished work to their teacher for review.
